Chronic Disease Management

Our Better Health programs include specialised clinics to improve patients' health with existing or potential chronic health issues.
Patients are encouraged to discuss their ongoing health management and concerns with their doctor or nurse practitioner. Nurse practitioners play a vital role in case management, focusing on symptom management—primarily pain relief—and providing emotional support while collaborating with other healthcare providers.
None of us can predict the future or what may happen to our health. In a crisis, your family may struggle to decide on the best treatment options for you.
We provide a link to help prepare an Advance Care Directive, ensuring that your wishes are clearly documented, to assist your family and healthcare providers in understanding your preferences when you cannot communicate.

Visits to Aged Care Facilities

Some general practitioners visit Banksia Lodge, Broulee, Illawarra Retirement Trust (IRT), Moruya, and Crown Gardens, Batemans Bay. Your GP and your Nurse Practitioner have full access to your medical record at the facility, and our practice nurses conduct comprehensive medical assessments as required.
  • Banksia Lodge: Dr Neil Starmer, Dr Neil Mackay, Theresa Pot NP – Thursday mornings until 1 pm
  • IRT Moruya: Dr Emily Goodwin, Theresa Pot NP – Wednesday afternoons from 1 pm

Home Visits

Certain doctors offer home visits for patients who are immobile, elderly, seriously ill, or in need of palliative care. Your doctor will take the time to discuss the arrangements with you and your family to ensure your needs are met. To further support these visits, we are pleased to have a nurse practitioner on our team.
Nurse practitioners play a crucial role in case management by concentrating on symptom relief—particularly pain management—and offering emotional support, while also collaborating with other healthcare professionals.
It is important to acknowledge that none of us can predict the future or foresee what changes may occur regarding our health.
In a crisis, your family may find it difficult to decide what treatment is best for you. To help your family and us know what you would want when you cannot tell us yourself, we have a link to prepare an Advance Care Directive so that your wishes are clearly recorded.
