If you would like to be bulk billed for eligible consultations, you need to complete an enduring Assignment of Benefit(AoB) form.
This form allows Medicare benefits to be paid directly to your doctor. You must also be registered with MyMedicare and have a preferred GP at our practice.

What do I need to do?

1. Register with MyMedicare if you have not already done so.
2. Download and complete the enduring AoB form.
3. Return your signed form by email or in person.

Who needs to complete the form?

Every current patient who would like to be bulk billed for eligible consultations must complete their own form.
Each member of a family must submit a separate signed form, including children who are current patients.

Why is the form required?

The enduring AoB form authorises Medicare to pay the benefit for an eligible consultation directly to your doctor.
Completing the form allows our practice to bulk bill eligible consultations, provided you are also registered with MyMedicare and have nominated a preferred GP at our practice.

What happens if I do not complete the form?

If we do not receive your completed form, you will be charged privately for the full cost of your consultation.

How long does the agreement last?

The agreement expires after 12 months.

How do I return the completed form?

You can return your completed and signed form:
• by email to admin@queenstreetmedical.com.au
• in person at the practice
If you are unsure what to do, please call us on 02 4474 2222.
