Over the past few years, we’ve seen big changes in how healthcare is delivered across Australia — from new ways of booking appointments to more options for accessing care. With so much happening, it’s important that we hear directly from the people who matter most: you.

At Queen Street Medical Centre, we’re committed to making your experience with us as clear, respectful, and supportive as possible. This short questionnaire is your chance to tell us what’s working well, what could be better, and how we can continue to shape our services around you and your family’s needs.

Why your feedback is important

Your feedback is completely anonymous and will be reviewed by CFEP Surveys, a professional survey company. This means your thoughts are secure, and we can focus on making real improvements. By sharing your experiences, you’re helping us identify areas for improvement and make changes that benefit everyone.

What the questionnaire covers

We’re keen to hear your thoughts on a few different things, including:
  • Booking Appointments: How do you feel about making appointments and chatting with a doctor or nurse over the phone when you need to?
  • Service Delivery: What do you think about home visits, after-hours service, and seeing your preferred doctor or nurse?
  • Consultation Experience: Are you comfortable in our consultation and waiting areas, and do you have the privacy you need?
  • Doctor/Nurse Interaction: How was your visit? Did the doctor or nurse listen well, explain things clearly, and make you feel respected?
  • Staff Conduct: How were you treated by our staff, and did we keep your personal information private?
  • Practice Information: Did you get enough information about fees, potential costs, and ways to stay healthy?
  • Care Coordination: How well did we coordinate your care with other healthcare providers, and did we respect your right to get a second opinion?
  • Overall Satisfaction: How happy are you with our practice, and do you have any suggestions for us?

Demographic information

We’re also asking for some general demographic details to better understand who’s taking part in the survey. This information will stay confidential and won’t be used to identify you.
